MEDICAL HISTORY AND PERMISSION FOR TREATMENT
Please bring the completed Medical History & Permission for Treatment Form with you to registration on Sunday. The form does notrequire a doctor’s signature or a physical exam, but it does require the signature of a parent or guardian. Do not mail the form to us—bring it with you to camp.
CAMPER CONDUCT CONTRACT
Please bring the completed Camper Conduct Contract with you to registration on Sunday. Both the camper and a parent or guardian should review and sign this document in advance. Do not mail the form—bring it with you to camp.
