Long-Term Key Request Form (Faculty and Staff)

This form is to be used ONLY by School of Music Faculty and Staff to gain access to Music Building spaces. Students who need keys should fill out the Long Term Key Request form.

You will receive a follow up email explaining how to pick up your key(s) and any other needed materials, if applicable.

All keys must be returned to Shar Joyner in Room 339 of the UNCG School of Music. If you will be keeping key(s) during the next access window, please fill out this form again.

Fall and Spring Access: returned or renewed by Spring Commencement (May)
Summer Access: returned or renewed by August 15

The lost key fee is $40.00 for each cut key (a.k.a. hard key).
The lost key fee is $55.00 for each intellikey. The $33.00 deposit will be put towards this fee and the student will be responsible for the remaining $22.00 of the lost key fee.

 Access to building space is intended for School of Music business, practice, and performances. The School of Music reserves the right to revoke privileges at any time.

What faculty/staff will need to complete this form

1. UNCG Student ID Number
2. UNCG Spartan ID Card Number
3. UNCG email address
4. Working Phone Number
5. Name and email address of Faculty/Staff member giving approval
6. ALL Room NUMBERS for which you are requesting a key

Keyholder Acknowledgement

Filling out this form implies understanding of the School of Music Building and University Policies and indicates an agreement to abide by the Code of Conduct. Specifically: The Keyholder agrees to not duplicate any keys issued to them. Keys shall not be loaned or transferred to anyone. Faculty and staff must return all access control keys issued to the Facilities Operations Customer Service Center upon separation from their department.
