This form is to be used ONLY by students who have received special permissions by a School of Music or CVPA Staff or Faculty Member to gain access to Music Building spaces (e.g. teaching assistants, student workers). Faculty and Staff who need their own keys should send an email to Emily Ford-Coates at firstname.lastname@example.org.
Once UNCG Facilities has fulfilled the request, you will receive an email stating how to and by when you can pick up your keys with a Photo ID. If you need access to the room before your key is ready, see the CVPA Front Desk (Room 220) for short-term key check-out.
All keys must be returned to the Emily Ford-Coates in Room 331 of the UNCG School of Music by the end of the final exam period within the school year they were issued. Failure to do so will result in a hold being place on the student’s account with the option to impose of a late fee on June 1st if the encumbrance has not been addressed. (For example: Keys issued in Fall 2018 or Spring 2019 will be due by May 9, 2019.)
The lost key fee is $40.00 for each cut key (a.k.a. hard key).
The lost key fee is $55.00 for each intellikey. The $33.00 deposit will be put towards this fee and the student will be responsible for the remaining $22.00 of the lost key fee.
The students are held to the student code of conduct regarding use of the facility space. Access to building space is intended for School of Music business, practice, and performances. The School of Music reserves the right to revoke privileges at any time.
Filling out this form implies understanding of the School of Music Building Policies regarding long-term check out of room keys and indicates an agreement to abide by the Student Code of Conduct and any policies set by the School of Music.
What a student will need to complete this form:
- UNCG Student ID Number
- UNCG Spartan ID Card Number
- UNCG email address
- Working Phone Number
- Name and email address of Faculty/Staff member giving approval
- ALL Room NUMBERS for which you are requesting a key
Filling out this form implies understanding of the School of Music Building and University Policies and indicates an agreement to abide by the Student Code of Conduct. Specifically: The Keyholder agrees to not duplicate any keys issued to them. Keys shall not be loaned or transferred to anyone. Faculty and staff must return all access control keys issued to the Facilities Operations Customer Service Center upon separation from their department.
